failed to :

perform or document weekly inspection on containers of hazardous waste in storage and daily inspections of the on site hazardous waste storage tank farm,

to determine the average volatile organic as required for the hazardous waste storage tank,

have an appropriate training program in place for facility personnel,

maintain the required documents and records for persons involved in hazardous waste management,

adhere to Contingency Plan requirements and to maintain in its files, a copy of the one-time written notice that included the information required in the Generator Paperwork Requirements Table,

stored hazardous waste without complying with the requirements set forth in Rules

and complete, sign and submit a permit application to the Hazardous Waste Section for a storage permit as required by the Rules.

stored and/or disposed of hazardous waste without complying with the Hazardous Waste Rules,

failed to have a permit to store and/or dispose of hazardous waste at the Jack’s Tire Service site.
