Failed to pay Small Quantity Generators Fees

Failed to:

• perform some weekly inspections;

• have a job title, a written job description, a written description of the type and amount of introductory and continuing;

• maintain training records for an employee who had hazardous waste management duties;

• list in the contingency plan, the capabilities of the emergency equipment, describe evacuation routes;

• redistribute to local emergency authorities an updated contingency plan;

• file an exception report for a manifest; and

• place in containers, universal waste lamps and the facility failed to properly label the universal waste lamps and batteries.

Failed to:

• maintain and operate the facility to minimize the possibility of a sudden or non-sudden release of hazardous waste constituents to air, soil or surface water which could threaten human health or the environment;

• immediately carry out the provisions of the contingency plan whenever there is a  release of hazardous waste or constituents which threatens or could threaten human health or the environment;

• not store a container of hazardous waste that is incompatible with any waste or material stored;

• complete inspection log book and place in designated log-book area;

• maintain aisle space of 24 inches; and

• clearly mark to identify container contents and the date each period of accumulation begins.

• waste paint related material was left on outside of container

• label containers with words “Hazardous Waste” or with other words that identify the contents.

• provide access to an internal alarm or emergency communication device

• label 8 containers with words “Hazardous Waste” or with other words that identify the contents;
• close container being used to accumulate universal waste lamps;
• label containers being used at the facility to accumulate universal waste lamps were not labeled “Universal Waste-Lamps,” or “Waste Lamp(s)” or “Used Lamp(s)”;
• demonstrate the length of time that the universal waste lamps had been accumulated from the date they became a waste or were received; and
• properly label 6 5-gallon containers and 1 3-gallon container of used oil with the words “Used Oil”.

• make hazardous waste determinations on numerous containers of chemicals;

• adequately mark two glass satellite accumulation containers with the words “Hazardous Waste” or with other words that identified the contents of the container; and

• label or mark one 55-gallon container of used oil with the words “Used Oil.”
