  • prepare hazardous waste manifests for shipments of hazardous waste;
  • close, labeled or mark a 30-gallon trash container with the words “Hazardous Waste”;
  • provide documentation of the refusal of State or local authorities to make the required arrangements was not available at the time of inspection;
  • name of the emergency coordinator and the location of fire and spill equipment were not posted next to the phone;
  • employees had not received adequate training;
  • clean up spills of hazardous waste from a trash container used to store hazardous waste;
  • determine if waste required treatment prior to being land disposed;
  • place used fluorescent tubes in containers;
  • mixed hazardous waste brake cleaner with used oil for disposal; and
  • label or mark a number of used oil containers with the words “Used Oil.
