  • provide documentation of the refusal of State or local authorities to make the required arrangements was not available at the time of inspection;

  • name of the emergency coordinator and the location of fire and spill equipment were not posted next to the phone;

  • employees had not received adequate training;

  • clean up spills of hazardous waste from a trash container used to store hazardous waste;

  • determine if waste required treatment prior to being land disposed;

  • place used fluorescent tubes in containers;

  • mixed hazardous waste brake cleaner with used oil for disposal;

  • label or mark a number of used oil containers with the words “Used Oil.

  • operated a hazardous waste storage facility by storing hazardous waste on site for greater than 180 days;
