Solid Waste Facility Reporting Forms
These forms are required of all solid waste facilities active in North Carolina between the dates of July 1, 2007 and June 30, 2008. The forms should be completed and returned no later than August 30, 2008. Questions regarding this form can be directed to your Regional Environmental Senior Specialist.

- Industrial Landfill (INDUS)
- Land Clearing and Inert Debris (LCID)
- Municipal Solid Waste Landfill (MSWLF)
- Mixed Waste Processing (MWP)
- Tire Landfill (TIRELF)
- Tire Treatment and Processing (TIRETP)
- Treatment and Processing (TP)
- Transfer Station (TRANSFER)
