Solid Waste Facility Reporting Forms

These forms are required of all solid waste facilities active in North Carolina between the dates of July 1, 2008 and June 30, 2009. Failure to report will result in enforcement action being taken.

1. The form is to be completed and returned no later than August 1, 2009.
2. Questions regarding this form can be directed to your Regional Environmental Senior Specialist.
3. The forms are Adobe (.pdf) and should be able to be saved locally with data.

Local Government Reporting Form

This form is required of all municipal and county governments in North Carolina. Failure to report will result in the community being exempt from receiving disposal tax distributions in the future. To view a list of towns required to report click here.

1. The form is to be completed and returned no later than August 15, 2009.

3. MS Word and Adobe versions are available and both should be able to be saved locally with data, but we prefer the form be filled out and filed using the submit button on the Adobe version.
